1. Intake letter is an informational letter reminding the client of the time, date, duration, location, contact information, what to expect and who to bring to the first appointment. PDF VERSION
2. No Show Letter is sent when a client fails to keep a scheduled appoint. It explains the policy of the project regarding missed appoints and offers a contact number to reschedule. PDF VERSION
3. No Treatment Letter is notification to the client regarding missed therapy sessions, options, and contact information. PDF VERSION
4. Intake Sheet is used as part of the Intake packet when a client first enters treatment. It is both an information/survey sheet used by the facility to see if a client is interested in participating in a study and if they meet general criteria for a specific study. PDF VERSION
